The following information is necessary to be collected from all staff for our records. It will let us know what size staff shirt to order for you, what days we will need to reserve a staff room for you, as well as information we would like to have on hand at the convention in the event of an emergency.

The following information will be kept confidential and not be made public. It will also not affect the decision to approve or deny your staff application.

Any parts of the section below not completed will need to be supplied within three days if you are approved to a staff position.

Hotel Request: All staff members are entitled to a free hotel room shared with up to three other staff members. If you wish to stay in one of our staff rooms, which nights would you need? (If you arrive Thursday and leave Monday, check Thursday through Sunday nights. If you leave Sunday, do not request Sunday night.)
